The Warren County Sheriffs Office is pleased to make available to the public all information not exempted from disclosure by the General Assembly of the State of Ohio.
The Warren County Sheriff's Office accepts incident and accident report requests by fax, mail or in person. To assist us with your request, please provide the following information:
- Date of incident
- Location
- Name of person on report
- Type of report
- Fax number or address to whom the report should be sent to.

You may personally come to the Warren County Sheriff's Office, 550 Justice Drive, Lebanon, Ohio 45036, Monday - Friday from 8:00 am to 7:00 pm to request copies of reports.
There is no charge for copies of the first five (5) pages; any additional pages will be copied at a cost of $0.05 per page.
Please allow a reasonable amount of time for your request to be processed.
